Self Referrals

You can access many free NHS health and wellbeing services directly without the need to see your GP first for a referral.

The service uses a range of specialists who can assess and treat your condition. ESMSK also provides care and support for rheumatology patients and those suffering with chronic pain. Patients can self-refer without seeing a clinician first.

One You East Sussex

Whether you’d like help to lose weight, stop smoking, drink less or get active, One You East Sussex is a FREE service to help you look at your lifestyle choices, put yourself first and do something about your own health. They provide tools, motivational support and encouragement every step of the way, to help improve your health right away.

Pregnancy

It’s important to see a midwife as early as possible to get the antenatal care and information you need to have a healthy pregnancy. You do not need to tell a GP you’re pregnant unless you’re taking medicine or have a long-term health condition.

You can refer yourself for pregnancy care from 5 weeks pregnant by completing a Maternity Self-Referral Form

If you cannot refer yourself online please contact the Midwifery Liaison Office on: 01892 638158 or 01892 633488
