Are you a Carer?
Do you look after someone who couldn’t manage without your help? Whether they need support due to illness, frailty, disability, mental health challenges, alcohol or drug dependency, difficulties or problems, if they rely on you, then you are a Carer.


Carers Identification Form
If you care for a relative or friend, formally or informally, we can record this on your medical records so our team is aware of your role and can provide appropriate support. If we know you are a carer we can:

  • Discuss the impact of your caring role on your mental or physical health
  • Invite you for a free flu jab each year
  • Provide you with general information and advice
  • Refer you to organisations and services offering specialised support
  • Try and offer flexible appointments to fit around your role

The easiest way to let us know you are a carer is by submitting our Carer Identification Form. Paper copies are also available at both surgeries.


Carers Lead
Our Care Coordinator, Emily Moore, is here to make sure your caring role is noted in your medical record and to help you access support – both in the surgery and through local services.

There is also a wealth of information on the NHS website about carers and caring, including carer’s assessments, support from local councils, respite care, and help for young carers.

We are committed to making our practice carer-friendly by:

  • Maintaining a Carers Register to ensure carers are identified and receive appropriate support
  • Having a dedicated Carers Lead
  • Training and encouraging all staff to understand carers’ roles and support them by signposting or making referrals to appropriate services
  • Offering flexible appointments to fit around carers’ commitments
  • Ensuring carers are invited annually for a free flu jab
  • Referring carers to our Social Prescriber for advice and support with local services, wellbeing, and practical help
  • Supporting national initiatives like Carers Week and Carers Rights Day
  • Sharing regular updates, information, and resources through our website and Facebook page
  • Encouraging carers to join our Patient Participation Group to share their experiences and views

Care For Carers (East Sussex registered patients)

Care for Carers East Sussex

Care for the Carers is an independent charity and the Carers Centre for East Sussex. Their team of staff and volunteers provides free practical and emotional advice by telephone, face-to-face, or online. They can connect you with other carers and help you navigate the range of services available locally. The charity also runs support groups, training sessions, and events across the county.

Involve Kent (Kent registered patients)

Involve Kent is a charity supporting carers across Kent. They offer free practical and emotional support through one-to-one sessions, peer groups, and wellbeing activities. They can help you access a Carer’s Assessment, register for the Kent Carer’s Emergency Card, and connect with local services and resources. Involve Kent also runs training, social groups, and events to help carers feel supported and less isolated.
