Reviews for Long-Term Conditions
Patients with long-term conditions are invited for reviews, often around their birthday month. These reviews help ensure routine checks are completed and that ongoing conditions such as asthma, COPD, diabetes or heart disease are appropriately monitored.
What this involves will depend on individual health needs and may include blood tests, monitoring checks or a medication review. Additional follow-up appointments may be arranged if required.
Patients with high blood pressure are encouraged to monitor readings at home and provide these for review. If a patient does not have a blood pressure monitor, one can be loaned for short-term use. Nurse appointments are available where home monitoring is not possible or further assessment is required.
NHS Health Check
The NHS Health Check is a free assessment of cardiovascular health. It helps identify whether a person may be at increased risk of conditions such as heart disease, stroke, diabetes or kidney disease.
During the appointment, height, weight and blood pressure are measured, and a cholesterol test is carried out. The Healthcare Assistant will also ask about lifestyle, family history and other risk factors before calculating a cardiovascular risk score and discussing the results.
Patients aged 40-74 who do not already have a diagnosed cardiovascular condition are invited for a free NHS Health Check every five years.
