We can only accept new patients who live in our practice area.
The practice serves the villages of Groombridge, Hartfield and surrounding areas. Please see the map within our Practice Leaflet.
How to register
You can register by completing a registration form which can be obtained from the practice. You will need to provide two sorts of identification;
1. Photo identification OR a previous medical card
2. A utility bill OR a credit card bill stating your local address.
In the case of a family one person's ID will suffice unless the names of the adults are different.
If you need to see a Doctor urgently before you can provide this information you will be registered as a temporary patient.
When you register, we will ask you to make an appointment to see the Healthcare Assistant for a new patient health check. At this appointment routine health checks (e.g.blood pressure) can be brought up to date and health promotion advice given. There will also be time for you to discuss any health care concerns that you might have.
Instructions on what to do with the registration form
Once you have been accepted as a patient your medical record will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
