If you would like to lodge a complaint, please make this in writing to the Practice Manager by letter or by using the form below. Press the 'Submit' button to send the email to us.
Once your email has been received, the Practice Manager will start the formal practice complaints procedure. You should receive an acknowledgement within two working days. We will aim to complete the investigation within 10 working days. If this should not be possible you will be kept informed of the reasons
